The following forms must be completed by a medical practitioner and parents:

• New student medical forms
• Returning student medical forms; students entering grades 3, 6, and 9 only
• Long term prescription medication form (e.g. for the treatment of ADHD).
• Anaphylaxis treatment plan. Students with Anaphylaxis must have an EpiPen available for use at all times whilst on campus and 2 EpiPens available for off campus trips.
• Asthma management plan for moderate to severe asthma. 

For the administration of short term over the counter or prescription medication in school or on trips:

• Short term medication administration form must be completed by parents.

NOTE: all medication supplied must be in its original packaging in English or Thai with the expiry date clearly visible. Medication prescribed by a doctor must have a prescription label attached.

For Health and Safety reasons we cannot accept or administer medication that does not meet this criteria.
